Beneficiary bank	:	Bank Handlowy w Warszawie S.A.
Beneficiary name	:	Dom Maklerski Banku Handlowego S.A.
Beneficiary address	:	ul. Senatorska 16, 00-923 Warszawa
Title of transfer	:	Crediting of the investment account (first and last name of the owner of the investment account)

* If a transfer is sent from outside the Republic of Poland, money account number should be preceding by the “PL” capital letters.
