Service Redemption Form (Options accounts only)

To redeem Driver's Edge rebates for automotive services:
(1)Use your Citi Driver's Edge Options Card to pay for any automotive services over $25.
(2) Complete the attached form and return it to us at the address below. Be sure to include a copy of the itemized automotive service receipt, with your Citi Driver's Edge Options account number circled, describing the automotive services performed on your vehicle.
(3) Your redeemed rebates will be posted to your account as a statement credit within two billing periods of the date we receive your completed form. Please note that you must return this form to us within 90 days of the date of Service and that any documents you submit cannot be returned. The minimum number of rebates that you can redeem is $25 and the maximum $5,000.

For Cardmembers Enrolled in Drive RebatesSM Feature: If the services described on this form were performed on the vehicle enrolled in the Drive Rebates feature, the mileage reflected on this form will be automatically submitted under that feature of the Driver's Edge Rebate program. Please note that you must include your VIN number on the form in order for your miles to be updated.You do not need to complete a separate Miles Submission form for the mileage reflected on this form.
