Service Redemption Form (Options accounts only)

To redeem Driver's Edge® rebates for automotive services*:
(1) Use your Citi ThankYouSM Card to pay for any automotive services over $25.
(2) Complete the attached form and return it to us at the address below. Be sure to include a copy of the itemized automotive service receipt, with your Citi ThankYou account number circled, describing the automotive services performed on your vehicle.
(3) Your redeemed rebates will be posted to your account as a statement credit within two billing periods of the date we receive your completed form. Please note that you must return this form to us within 90 days of the date of Service and that any documents you submit cannot be returned. The minimum number of rebates that you can redeem is $25 and the maximum $5,000.
