• Previously declined applicants of a Citibank Credit Card can only reapply after 6 months.
• Please fill in correctly all white spaces provided, placing N.A. where Not Applicable.
• Applications not accompanied by documents or with incomplete information will not be processed.
• The following information is required for online applications. Please have these ready before proceeding to ensure a more convenient online application experience:
  a) an e-mail address - If you do not have one yet, please create one through free e-mail account providers on the internet.
  b) your Tax Identification Number (TIN)

Document Requirements:

If employed, submit a copy of ANY ONE of the following:

• Income Tax Return with BIR or Bank Stamp
• W-2
• Latest Pay Slip (within last 3 months)
• Last month's credit card/s statements (you must have had the credit card/s for at least 12 months and credit card/s must be of current standing)
• Car Registration* with Official Receipt for car models not older than 36 months
• Certificate of Employment stating income (original)

If self-employed, proprietor or businessman, submit a photocopy of any of the following:

• Audited Financial Statement with BIR or Bank Stamp
• Income Tax Return with BIR or Bank Stamp
• W-2
• Last 2 month's statements from the same credit card/s (you must have had the credit card/s for at least 12 months and credit card/s must be of current standing)
• Car Registration* with Official Receipt for car models not older than 36 months

  *Not valid for residents of Davao City and Cagayan de Oro
