What is a Life Plus Plan?
Life Plus is a Comprehensive Life Protection Plan underwritten by ALICO (American Life Insurance Company (Pakistan) Limited). Life Plus Plan requires no medical check-up. It provides the family with "Rs. 1,000,000" in any event resulting in Loss of Life or Permanent Total Disability. It is affordable and is offered to Citibank Cardmembers at only Rs. 250/month for a Life cover and Rs.325/month for both Life and Permanent Total Disability cover.


Life Plus Plan Features
Life Plus covers both accidental and nonaccidental risks to life and provides a 24-hour worldwide protection against the following:
1. Death
2. Permanent Total Disability


Death:
If a Citibank Cardmember dies while insured under this plan, ALICO (American Life Insurance Company (Pakistan) Limited) will pay Rs. 1,000,000 to the beneficiary(ies).


Permanent Total Disability:
If a Citibank Cardmember becomes Permanently Total Disabled while insured under this plan, ALICO (American Life Insurance Company (Pakistan) Limited) will pay Rs. 1,000,000 to the policy holder/beneficiary(ies) (Permanent Total Disability benefit is offered with the Death benefit).

Eligibility
Citibank Cardmembers between the age of 18 and 60 are eligible for enrollment under Life Plus Plan (renewable up to 65th Birthday).

Cancellation
The policy may be cancelled at customer's request at any time by calling CitiPhone at 111 222 222 or by writing to us at the following address:

Once the policy is cancelled, the customer will not be liable to pay the remaining premium installments.


Free Look Period
Life Plus offers a Free Look period of 30 days in which the Citibank Cardmember can cancel the policy by calling Citibank
at 111 222 222 and get the full premium refunded.


Exclusions
General exclusions of Life Plus Plan are as follows:
• Death/Disability due to non-accidental reasons within 18 months due to a pre-existing condition
• Suicide within 18 months from the enrollment date
• AIDS and/or AIDS related diseases
• Actively participaiting in war

Following exclusions apply to only Permanent Total Disability claims:
• Intentionally self-inflicted injury or any attempt of self destruction
• Racing on wheels or on horses or in boats, or water skiing
• Any attempt of an assault or any unlawful act
• Service, travel or flight in any kind of aircraft as an operator, member of crew, except as a fare-paying passenger
• In case the insured customer is female, pregnancy, delivery and/or abortion will not be considered

Claims Handling
You should contact the Claims Manager of ALICO (American Life Insurance Company (Pakistan) Limited) at 021-111 111 711 from
anywhere in Pakistan. ALICO will settle the claim within 15 working days upon completion of the claim documents.


For Further Inquiries/Complaints
For any inquiries/complaints, please call CitiPhone at 111 222 222. Your request will be forwarded to our Inquiry Management and Problem Resolution Department and will be subsequently escalated to higher levels for early resolution.

A grace period of thirty (30) days following each Premium Due Date shall be allowed for the payment of outstanding Monthly Premium in respect of each Insured Borrower after the first Policy Month. If any outstanding Monthly Premium in respect of an Insured Borrower is not received before the expiration of the grace period, the insurance coverage extended to such Insured Borrower in terms of this Policy shall automatically terminate at the expiration of such grace period and the Policyholder shall not be liable to the Insurance Company in such regard.

  • The Life Plus Plan is underwritten and issued by ALICO (American Life Insurance Company (Pakistan) Limited).
  • Life Plus Plan is subject to ALICO’s (American Life Insurance Company (Pakistan) Limited’s) Terms & Conditions.
  • Citibank, N.A. will not be responsible in any manner if your application or claim is rejected.
  • Citibank, N.A. does not offer insurance advice or underwrite or issue insurance policies.
  • Premiums made by the policy holder will be paid to the underwriter. Premium amounts for this insurance plan are not a bank deposit, or other obligation of, or guarantee by Citibank, N.A., Citigroup or any other affiliates.
  • Purchase of insurance is optional and not required to obtain/maintain credit.
