List of documents required for your Credit Card Application:

If you are

• Salaried
• Self Employed

If salaried then
You will be required to furnish the following documents:

• Computerized National Identity Card copy
• Proof of Employment:
  • Certificate of employment including length and status of employment or
  • Tax documents
• Proof of Income:
  • Salary slip and/or Salary Certificate with complete details of allowances and deduction and/or
  • last six months personal bank statement and/or
  • last year Tax Assessment Order or
  • IT-30 Form.

The above requirement may vary depending on the company you work for.

If self employed then in case of

• Sole Proprietor
• Partnership
• Private Limited Company

Sole Proprietor

• For Proof of Proprietorship (any of the following)
  • Bank Certificate (Sole Proprieter Certificate in case bank statement is on company name)
  • Income Tax Assessment Order
  • Lease / Rent Agreement
  • Sale Agreement

• Proof of Income (any of the following)
  • Tax assessment order (personal or in company's name)
  • Tax return and tax payment receipt
  • Personal or company bank statements for the last 6 months.

Partnership

• Proof of Business (any of the following)
  • Partnership Deed (Stamped by Notory Public)
  • Personal tax assessment order
  • Bank certificate stating joint signatory for company accounts is acceptable in case of a blood relation, where partnership deed is not available.
• Proof of Income (any of the following)
  • Tax assessment order, personal or in company's name
  • Tax return and tax payment receipt.
  • Personal bank statement or company bank statement with proof of partnership.
    

Private Limited Company

• Proof Of Business
  • Memorandum and Article of Association
• Proof Of Income (any of the following)
  • Company income tax assessment order
  • Personal income tax assessment order
  • Personal Bank statement (Used as a surrogate of income)
  • Tax returns and tax payment receipt
    

Note

1. For application with bank statements, following type of accounts will be accepted.
• Personal
• Joint
• Foreign Currency
• Business (Sole Proprietorship with ownership proof)
• Company Account (Partnership concern)
2. Citibank may in its sole discretion decide the documentation and income requirements.
3. The copies of the documents submitted in respect of the application shall not be returned
