Who to Call
- The Cardmember must immediately contact Citicorp Insurance Services, Inc. (CISI) to report the loss and obtain a claim form - Inside the U.S., the Cardmembers should call 1-800-613-5975. Outside the U.S., Cardmembers should call collect 1-817-348-7166
- The police to obtain an accident report (if applicable)
- The rental agency to notify them the that an accident has occurred
Documents Required During Filing*
- Signed and completed claim form
- The front and back of the Cardmember's Rental Agreement
- Copy of the Diners Club charge receipt to verify that the entire rental fee was charged to a Diners Club Account
- Itemized repair bill from a qualified body shop indicating the actual repair cost to the rental vehicle (the rental agency should provide)
- The police or accident report (if applicable)
- Copy of the Cardmember's drivers license (front and back)

*Important Notice - The completed claim form must be received by CISI within 90 calendar days from the accident, damage, or loss to qualify for consideration as a valid claim. If the claim is not submitted during the 90 day time period, it will not be considered for payment. If the additional required supporting documents are temporarily unavailable during the 90-day limit, the Cardmember has up to six months to supply supporting documents to CISI.
Exclusions:
- Some countries' local laws or agency practice may prohibit the declining of CDW at the rental agency. In these cases, should the Cardmember get into an accident, Diners Club coverage becomes secondary and will cover any deductible for which the Cardmember is responsible.
