3 last current account banking statements from your bank/banks (1 statement if you receive statements on a quarterly basis)
The list of regular payments which are not included in your account statements (e.g. yearly, half-yearly standing orders, etc.)
Address of your employer and pay-roll date
If you use SIPO then your connection number - "spojovací číslo"
If you use direct debit then names and addresses of receiving companies (mobile operators, utility providers, insurance companies, etc.) including your customer numbers or insurance policy numbers.
